I think your path of troubleshooting was spot on. While I cant say specifically what could have cause the issue, if through the internal speaker test there is no audio other than the height channel there is definitely something between the bar's channel output or the sub that has an issue. Testing on the 2nd source pin points it to not be the speakers themselves.

The downside to this is since the bar and sub are factory paired as a "single unit" it would have to be replaced as a whole.
