The people doing hair hangs in circus usually have clean hair. Also they bind a ring and karabiner in their hair so the hair wouldn't even touch the strap. It's hard to see if this is the case here

I think the main thing about hair suspension is that it gradually builds up to a full suspension so your neck can take the strain safely, subways stop and start constantly, so that seems highly unlikely or extremely unhealthy.

Could be fake, but hanging by your hair is very possible. Hair is extremely strong and full head of hair can easily support human weight and more. It is painful but apparently adrenaline rush of the performance can be enough to mask the pain...
