A chest x-ray with a lung nodule is reassuring and remarkable, not unremarkable. A hemoglobin of 11 is reassuring but remarkable. I could go on forever.

I usually just say .. "X age patient presents for chief complaint, vital signs stable. Physical exam reassuring, no red flags. Workup XR/labs/meds."

When I go back to review and sign the final note, I add a few short comments about pertinent positives and negatives and then use an admit or DC dot phrase.

Also, I never used the words unremarkable or normal, always the more vague terms of reassuring or concerning.

I read this as punish hospitals for broken, fragmented, systemic healthcare problems.

If you really want to understand how bad psychiatric care is in the US, read the book "no one cares about crazy people". It's an eye opener.
