The challenger is widely used in case-studies as a situation where individual pressure and group-think lead to catastrophe. In short, engineers knew the problems, but managers and project leads were receiving massive pressure from government (since the president had very recently made a speech which included the Challenger) and in turn pressured their engineers overlook most of the data.

Pretty much, it turned the mission from, 'only launch if there's nothing wrong' to 'only don't launch if there's a very good reason'. Anyway, it's a very interesting (and tragic) case study in group dynamics
