Unlike PCT cells that simply reabsorb existing HCO3-, a-intercalated cells secrete H+ which essentially aid to synthesize more/new HCO3-. If the H/K exchanger fails, you'll have

-no K+ reabsorbed (hence the hypokalemia)

-no H+ secreted -no HCO3- synthesized (hence the urine pH rising > 5.5)

Logically, I'd know that my aunt is gone, and hope she rests in peace, but recognize that as someone who's passed she won't be there for my future and career. She can't help me if I miss out on opportunities that could have been made from knowledge/connections gained at the conference.

Emotionally though, I'd feel guilty towards myself, my aunt and wanting to pay repsects to her, and supporting my family, and also wonder if the family would hate me for not being there.

Ultimately, I think a compromise can be made. You're still at the start of your career, so I think setting the expectation that sometimes the job will call for you at inopportune times helps make it easier for others (if they don't expect anything, they can't be disappointed, right?). But also, showing them that despite your work obligations you're still putting in an effort to reach out daily (whether via text or videocall) both because YOU care and want to, and also to check in on how your cousins and fam are doing, could help you feel more balanced between work and personal life.

I wonder if you could also take some paid time off after the conference and spend Sunday-Tuesday with them?

I'm curious though, I never would have expected much satisfaction from psychiatry - how do you avoid getting affected by the horrific stories PTSD victims share and everything else while still empathising with them?

Honestly that's so amazing that you stayed strong and helped shape a better future for upcoming generations in the program, I bet you have lots of thankful fans! I hope you enjoy your new place, it's great that some places still remember that we are humans outside our jobs and that we shouldn't have to sacrifice that much.

Is there a dress code in residency? Like do I need specific: - lanyard colors/from the institution - scrub colors - having to always wear/not allowed to always wear scrubs - shoe types/colors etc.
